Corneal collagen cross-linking is a technique which uses UV light procedhre a photosensitizer to strengthen chemical bonds in the cornea. The goal of the treatment is to halt progressive and irregular changes in corneal shape known as ectasia.

The most common form of ectasia is keratoconus and less often ectasia is seen after laser vision correction such as LASIK. Cross-linking of collagen refers to the ability of collagen fibrils to form strong chemical bonds with adjacent fibrils. In the cornea, collagen cross-linking procdure naturally with aging due to an oxidative deamination reaction that takes place within the end chains of the collagen.

It has been hypothesized that this natural cross-linkage of collagen explains why keratoectasia corneal ectasia often progresses most rapidly in adolescence or early adulthood but tends to stabilize in patients after middle-age.

In addition to the cross-linking that occurs commonly with corneal maturation, there are several other pathways that can lead to crosslinkage. Glycation refers to a reaction seen predominantly in diabetics that can lead to the formation of additional bonds between collagen. In the pathway most relevant to this topic, oxidation has been shown to be able to trigger corneal crosslinkage through the release of oxygen free radicals.

The bases for the currently employed corneal collagen cross-linking techniques were developed in Europe by researchers at the University of Dresden in the late ‘s. UV light was used to induce collagen cross-linking in riboflavin soaked porcine and rabbit corneas via the oxidation pathway. The resultant corneas were shown to be stiffer and more resistant to enzymatic digestion.

Investigation also proved that the treated corneas contained higher molecular weight polymers of collagen due to fibril crosslinking. Human studies of UV-induced corneal cross-linking began in in Dresden, and early results were promising. The initial pilot study enrolled 16 patients with rapidly progressing keratoconus and all of the patients stopped progressing after treatment.

There were no reported complications. In lateorphan drug status was awarded by the FDA to Avedro for its formulation of riboflavin ophthalmic solution to be used in conjunction with the company’s particular UVA irradiation system. The primary purpose of crosslinking is to halt pprocedure progression of ectasia. Likewise, the ;rocedure candidate for this therapy is an individual with a progressive ectatic disease of the cornea.

Corneal Collagen Cross-Linking

The most common indication is keratoconus. There currently are no definitive criteria for progression, but parameters to consider are change in refraction including astigmatismuncorrected visual acuity, best corrected visual acuity, and corneal shape topography and tomography. The primary goal of the first stage of therapy is to allow riboflavin to diffuse into the cornea.

While there are several variations on the techniques used to accomplish this, all entail either removing or weakening the epithelial barrier of the cornea. In all instances the patient is first given anesthetic drops. Some ophthalmologists will also give preoperative antibiotics. A lid speculum is placed. After disrupting the epithelium, drops of riboflavin 0. After adequate riboflavin absorption, the patient is positioned with the UV light typically um at a small distance cm from the corneal apex for 30 minutes.

Following irradiation, antibiotic drops are given and a bandage contact lens is typically placed.

The patient is given antibiotic drops to use postoperatively times daily. In the video shown, anesthetic drops are given, then the speculum is placed and the epithelium is removed.

Next, drops of riboflavin are administered, followed by Procesure exposure. The observational evidence for the role of corneal crosslinking has been strong.

This data is also supported by several well designed randomized controlled clinical trials The most consistent finding of observational and randomized controlled studies has been that corneal crosslinking induces a c3e decrease in keratometry values that tends to be maintained over at least a year. This is an important finding, as in progressive keratoconus keratometry typically rises over time and is a marker of disease progression.

While the goal of therapy is to either halt or reverse a progressive condition keratoconus or ectasia the various studies have not all clearly defined “progression”. Proceduee fact, many studies have either failed to define this starting point of enrollment eyes proccedure “progressive” disease or have defined it in a way that may not be acceptable to the ophthalmology community. For each study listed below, the definition of progression is emphasized due to its importance in our understanding of the resultant data.

The strength of this study is its large sample size at 1 year. The weaknesses are its poor definition of the disease being treated a major flaw and poor sample size after 1 year. The strength of this study is also its sample size at 1 year.

Similar to the Dresden study, its interpretation and application to a wider set of patients is limited by a poorly defined patient population. There is also a small sample size at 4 procerure.

Only states that it was defined “clinically and instrumentally within 6 months”. This ongoing study has the best published study design and definition of progression to date. The researchers are looking at patients with clearly defined progressive keratoconus and will follow them for 5 years. The three-year data was published in April and is described below.

Chapter 1 The Opinionated Knitter Most people have an obsession; mine is knitting. Your hobby may be pie-baking, playing the piano, or potbelly-stove collecting, and you can sympathize with my enthusiasm, having an obsession of your own.

Will you forgive my single-mindedness, and my tendency to see knitting in everything? Carvings and sculpture remind me only of Aran and other textured designs; when I see a beautiful print my first thought is how it would adapt to color pattern knitting; confronted by a new fashion, I immediately start drawing in the air with my forefinger to see if it would suit itself to knitting, and if so, how — which way the grain should run, if the shape could be knitted in, and what stitch would be most effective.

So please bear with me, and put up with my opinionated, nay, sometimes cantankerous attitude. I feel strongly about knitting.

What follows is an attempt to explain some of the ideas that come while designing knitted garments, and during the pleasant hours spent working on the plain straight pieces. I am taking it for granted that you are already familiar with the rudiments of the craft. Properly practiced, knitting soothes the troubled spirit, and it doesn’t hurt the untroubled spirit either. When Teqrs say properly practiced, I mean executed in a relaxed manner, without anxiety, strain, or tension, but with confidence, inventiveness, pleasure, and ultimate pride.

If you hate to knit, why, bless you, don’t; follow your secret heart and take up something else. But if you start out knitting with enjoyment, you will probably continue in this pleasant path. Consider the agreeable material and tools. I admit to a rooted preference for wool. There are scientific reasons why wool is the best material for knitting, and into these I will not go. I only know that it is warm, beautiful, and durable. It goes, “Never wind your wool into a tight hard ball, kniting this will make it stretched and taut. You knit it up into a sweater. Then it gets dirty and you wash it. The wool, encouraged by the dampness, goes back to its original unstretched state, with the result knutting the whole sweater shrinks.


It has nothing to do with the thickness of yarn, except in a relative way, and everything to do with its construction. A ply is a elizabehh of wool.

Two, three, four, or more strands are twisted together to make 2-ply, 3-ply, 4-ply, or many-plied wool. Since the strands or plies can be of any thickness, it is clear that the thickness of wool does not depend on the number of plies but on the thickness of the individual ply.

When wondering how much wool to buy, ask the saleslady. She knows by experience. If she doesn’t know and isn’t interested, go to another store. In fact, start off by going to the best specialty yarn shop or good department store that you can find.

It is not wise to shop around for cheap wool unless you kniting very experienced, or are willing to risk spending hours of work on an object that will shrink, fade, elizqbeth run. A well-made sweater, knitted with good will and good wool, is beyond price; why try to save a dollar on the material?
